Job: CARE COORDINATOR - BILINGUAL

Jobing Description

Responsible for collaborating with the interdisciplinary medical home care team in the provision of patient care, working with selected patients assigned to the medical home care team and primary care operations, and for performing an initial clinical review at the point of patient enrollment. In addition, the role integrates and coordinates access and utilization management, proactive patient panel management, care facilitation and treatment planning functions.

Upholds the standards of the system-wide customer service program.

Job Qualifications (required):
RN License State of Ohio
Degree/Diploma in Nursing
Minimum of five years clinical experience
Ability to work independently and as a member of a multidisciplinary team
Demonstrates excellent interpersonal, verbal/written communication and delegation and problem-solving skills.
Demonstrates strong analytical, data management and computer skills
Demonstrates strong leadership, organizational and time management skills

Job Qualifications (preferred):
Bachelors of Science in Nursing and/or Master s Degree
Two years experience with case management, access management, home-health care, discharge planning or related experience
Case Management Certification
Experience with case management, access management, home health care, psychiatric case management, discharge planning or related experience.
Knowledge of EPIC functionality.
Knowledge of case management software.
