Description
Clinically progressive, forward thinking pharmacy is recruiting for a clinical pharmacist at our innovative Outpatient Medication Therapy Management (MTM) Clinics. The Medication Therapies Center includes the following MTM clinics: Anticoagulation, Heart Failure, COPD, Diabetes, Hyperlipidemia, Fall Risk Reduction, Transitions of Care, and Drug Therapy Review/ Medication Adherence. Additional MTM clinics will be developed in the future.

MTM services focus on educating patients about how to properly use their medications, monitors the patients’ responses to therapy, and manages/adjusts therapy on an ongoing basis utilizing a collaborative agreement with referring physicians. MTM and chronic disease management services are provided in the outpatient Medication Therapies Center by a clinical team that includes Clinical Pharmacists, Nurse Practitioners and pharmacy technicians.

This is an excellent opportunity to practice clinical pharmacy the way it is meant to be practiced … with emphasis on fully utilizing your clinical training/skills in the care and teaching of patients. We are looking for candidates with strong medication management skills who also have excellent interpersonal communication skills and work very well in a team environment.

The department of Pharmacy at Memorial Hospital is a leading innovator in the provision of clinical pharmacy services. The hospital is a short 20 minute drive from northwest Columbus.


Requirements
Must be eligible for and maintain licensure as a pharmacist in Ohio. 2-5 years of relevant clinical pharmacy practice experience and/or completion of a clinical practice residency preferred.
